About This Privacy Policy

The Hospice Foundation of Martin & St. Lucie, Inc. (d/b/a Treasure Health) ("Treasure Health," "we," "us," or "our") respects your privacy and is committed to protecting the personal information of everyone who visits treasurehealth.org (the "Site") or interacts with our online services. This Privacy Policy explains what information we collect, how we use and share it, and the choices you have. By using the Site, you agree to the practices described in this Policy.

1. Information We Collect

We collect information in the following ways:

A. Information You Provide Directly

  • Contact information, such as your name, mailing address, email address, and phone number, when you fill out a contact form, request information, or subscribe to our newsletter.
  • Donation and payment information, such as billing name and address, payment card or bank details, and donation amount, when you make a gift or payment online. Donations are processed through Blackbaud, our third-party payment processor; payment card details are handled by Blackbaud and are not stored on our own servers.
  • Care inquiry and referral information, such as details about yourself or a loved one submitted through a referral, admissions, or “request care” form. This may include limited health-related information (for example, a diagnosis, prognosis, or physician name) that you choose to share so that we can respond to your inquiry. To the extent this information constitutes protected health information (as described in Section 4 below), our collection, use, and disclosure of it is governed by our Notice of Privacy Practices and not by this Policy.
  • Volunteer, employment, or event-registration information submitted through applicable forms.
  • Any other information you choose to provide when corresponding with us by email, phone, or web form.

B. Information Collected Automatically

When you visit the Site, we and our service providers may automatically collect certain information using cookies, web beacons, and similar tracking technologies, including:

  • Device and usage data such as IP address, browser type, operating system, referring/exit pages, and the dates and times of your visit.
  • Analytics data collected through Google Analytics and Google Tag Manager, which help us understand how visitors use the Site.
  • Advertising identifiers and interaction data collected through Google Ads and the Meta (Facebook/Instagram) pixel, which we use to measure the effectiveness of our outreach and to show Treasure Health content on Google, Facebook, Instagram, and other websites or platforms.

You can control cookies through your browser settings, including by disabling or deleting them; doing so may affect how portions of the Site function.

2. How We Use Your Information

We use the information we collect to:

  • Respond to inquiries, referrals, and requests for information or care;
  • Process and acknowledge donations and payments, and issue tax receipts;
  • Send newsletters, updates, and communications about our programs and services, where you have opted in or as otherwise permitted by law;
  • Operate, maintain, and improve the Site and understand how it is used;
  • Measure and improve the effectiveness of our advertising and outreach;
  • Comply with legal, accounting, and regulatory requirements; and
  • Protect the security and integrity of our Site, organization, and the people we serve.

3. How We Share Information

We do not sell your personal information. We may share information in the following ways:

  • With service providers who perform functions on our behalf, such as Blackbaud for donation and payment processing, email/newsletter delivery, website hosting, and Google and Meta for analytics and advertising services, each under obligations to protect your information;
  • With affiliated hospice and healthcare providers operating under the Treasure Health name, where necessary to respond to a care inquiry or referral;
  • With professional advisors (such as legal, accounting, or auditing firms) as needed;
  • With government authorities or other third parties when required by law, subpoena, or court order, or to protect the rights, property, or safety of Treasure Health, our patients, or others;
  • With a successor organization in connection with a merger, reorganization, or similar transaction involving Treasure Health;
  • With any other person or entity where you consent to the disclosure; and
  • For any other purpose disclosed by us when you provide the personal data or for any other purpose we deem necessary, including to protect the health or safety of others.

Health Information, Cookies, and Advertising

4. Protected Health Information Submitted Through the Site

Treasure Health is a licensed hospice and palliative care provider and, as a health care provider that engages in HIPAA-covered transactions, is a “covered entity” under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"). Protected health information ("PHI") that Treasure Health creates or receives in connection with the clinical hospice, palliative care, or bereavement services we provide — including health-related information submitted through a referral, admissions, or “request care” form described in Section 1.A above — is governed by our Notice of Privacy Practices, issued in accordance with HIPAA, and not by this Policy. Our Notice of Privacy Practices and at our facilities, and describes how we may use and disclose PHI, and your rights with respect to that information. Consistent with our Cookie Policy, our website analytics and advertising technologies are not used on pages where you submit or view health information, and we do not transmit PHI to Google, Meta, or other third-party analytics or advertising providers. If you have questions about how health information is handled in connection with your or a loved one’s care, please contact us using the information in Section 12 below.

5. Cookies and Online Advertising

The Site uses Google Analytics, Google Tag Manager, Google Ads, and the Meta pixel to understand Site usage, measure the performance of our advertising, and deliver Treasure Health messages to you on other websites and platforms such as Google, Facebook, and Instagram. These technologies are scoped to run only on unauthenticated marketing and educational pages of the Site, and do not run on pages where you submit or view health information. You can learn more about, and opt out of, Google's use of data through Google's Ads Settings (adssettings.google.com) and by installing the Google Analytics opt-out browser add-on. You can control how Meta uses your data for ads through your Facebook or Instagram ad preferences. You may also opt out of interest-based advertising generally through the Digital Advertising Alliance's opt-out page (optout.aboutads.info) or your browser and device settings. Your Choices and Our Practices

  • Email communications: You may unsubscribe from our newsletter or email communications at any time using the “unsubscribe” link included in each email, or by contacting us directly.
  • Cookies: You may adjust your browser settings to refuse or delete cookies, though some Site features may not function properly as a result.
  • Donor communications: You may request that we not contact you regarding future donation opportunities by contacting us as described below.
  • Access and correction: You may contact us to ask about, update, or correct personal information you have previously provided.

7. Children's Privacy

The Site is intended for a general adult audience and is not directed to children under 13. We do not knowingly collect, use, or disclose personal information from children under 13. If you believe a child has provided us with personal information, please contact us so we can remove it.

8. Data Security and Retention

We use commercially reasonable administrative, technical, and physical safeguards designed to protect personal information we collect and store from unauthorized access, use, or disclosure. No method of transmission or storage is completely secure, however, and we cannot guarantee absolute security. We retain personal information for as long as reasonably necessary to fulfill the purposes described in this Policy, including to comply with legal, accounting, and reporting requirements.

9. Third-Party Links

The Site may contain links that will let you leave the Site and access another third-party website, including payment processors and social media platforms. Linked websites are not under our control. This Policy does not apply to those third-party sites, and we encourage you to review their privacy policies before providing any personal information. We accept no responsibility or liability for these other websites.

10. Florida and U.S. Residents

Treasure Health operates in Florida and generally directs this Site to visitors within the United States. We process personal information in accordance with applicable U.S. and Florida law. If you are located outside the United States and choose to use the Site, you understand that your information will be transferred to and processed in the United States.

11. Changes to This Policy

We may update this Privacy Policy from time to time to reflect changes in our practices or for other operational, legal, or regulatory reasons. The updated Policy will be posted on this page with a revised effective date. We encourage you to review this Policy periodically.
